Advisory Board Member - Healthcare AI

Talamel Health Technologies is developing a Care Continuum platform called Cohezon, which connects hospitals, post-acute providers, primary care physicians, and patients across the post-hospital journey. The platform addresses care transition failures that drive significant costs through readmissions, delays, and discharge bottlenecks.

This advisory board role is designed for experienced leaders in healthcare delivery, care management, or enterprise health software. Advisors contribute strategically on a flexible, part-time basis, with no day-to-day execution responsibilities. Compensation is equity-based, aligned with long-term value creation.

• Provide strategic guidance on care transitions, care management, and healthcare AI adoption
• Advise on product-market fit, clinical workflows, and enterprise buyer requirements
• Support go-to-market strategy targeting hospitals, health systems, and post-acute networks
• Make targeted introductions to prospective customers, partners, or advisors
• Participate in occasional strategy calls on a flexible, part-time basis

• Experience in hospital or health-system leadership, particularly in care management, case management, or transitions of care
• Background in post-acute operations including skilled nursing, home health, or hospice
• Experience in payor or value-based care operations, such as ACOs, MA plans, or provider networks
• Track record in healthcare SaaS or enterprise AI scaling, or selling into health systems
• Strong professional network relevant to healthcare enterprise buyers and partners
